Medicare Advantage vs Medigap: How the Costs Really Differ

Medicare Advantage replaces Original Medicare with a private plan that has a network and a yearly out-of-pocket limit. Medigap sits on top of Original Medicare, pays part or all of what Medicare leaves behind, and has no network. Advantage often costs less each month, while Medigap makes doctor and hospital costs more predictable.

The comparison in one table

Medicare AdvantageMedigap with Original Medicare
Monthly premiumVaries by plan. Many plans charge no premium beyond your Part B premium.A separate premium on top of your Part B premium. It looks higher month to month.
Doctor and hospital costsCopays and coinsurance set by the plan.Covers part or all of Original Medicare's cost sharing, depending on the plan letter.
Yearly out-of-pocket limitYes, set by the plan.Original Medicare has no cap. The plan letter you choose decides what is left for you to pay.
NetworkHMO or PPO network. No network. Any provider that accepts Medicare, nationwide.
Prescription drugsMany plans include drug coverage. Not included. You add a standalone Part D plan.
Dental, vision, hearingOften included as extra benefits. Not included.
Changing laterAnnual Enrollment (Oct 15 to Dec 7), plus Jan 1 to Mar 31 if you are already enrolled. An insurer can ask health questions outside open enrollment. California's birthday rule is one exception.

You cannot have both at once

Medigap works with Original Medicare. If you are in a Medicare Advantage plan, you generally cannot be sold a Medigap policy. If you move from Advantage back to Original Medicare and want Medigap, the insurer may ask health questions unless you have a guaranteed right to buy.

Who tends to lean each way

Advantage tends to fit someone who wants a lower monthly premium, is comfortable using a network, and likes drug coverage and extras bundled into one plan.

Medigap tends to fit someone who wants to keep any doctor who accepts Medicare, splits time between states or travels often, and prefers to know what a hospital stay will cost before it happens.

Neither wins for everyone. The lower premium and the network rule are both real. Your own doctors and prescriptions decide which costs less for you.

Questions that settle it

  1. Do your doctors and hospital take the Advantage plan you are looking at?
  2. How often do you see specialists, and do you mind a referral step?
  3. Do you spend months at a time outside California?
  4. Would you rather pay a higher fixed premium or accept copays that vary with how much care you use?
  5. If you wanted to switch later, would health questions be a problem for you?

Frequently asked questions

Which costs less, Medicare Advantage or Medigap?

It depends on how much care you use. Advantage usually has the lower monthly premium. Medigap has the higher premium but makes visit and hospital costs more predictable.

Can I have a Medigap policy and a Medicare Advantage plan?

Generally no. Medigap pairs with Original Medicare and generally cannot be sold to someone who is in a Medicare Advantage plan.

Does Medigap cover prescription drugs?

No. If you have Original Medicare with Medigap, you add a standalone Part D plan for prescription drugs.

Can I switch from Medicare Advantage to Medigap?

You can return to Original Medicare during Annual Enrollment or the January to March window. Buying Medigap afterward may involve health questions, so ask about your rights before you leave your Advantage plan.
